[ ] By signing this Entry Form, the Rural American Idol contestant and/or contestant parent or guardian, acknowledges that the non-profit art organization of P.O.I.N.T. is NOT responsible for any prizes promised or offered by anyone, except the chance for the qualified contestants to compete in the scheduled Rural American Idol Contest for a win and gain a spot in the Final Rural American Idol Competition (TBA).

PROMOTIONAL AUTHORIZATION:
[ ] By signing this Entry Form, the authorization below is for the promotion of the Rural American Idol contest participant and the RURAL AMERICAN IDOL Program initiated by the art organization called People Organizing Ideal Neighborhoods Today, Inc.(a.k.a. P.O.I.N.T.) and its monthly show, RURAL AMERICAN MUSIC REVUE.

I, ________________________________, grant to P.O.I.N.T. the right to utilize the act's name, likeness, image, voice, appearance, and performance in perpetuity worldwide for advertising, trade or any other lawful promotional purposes whatsoever, in any and all media now known or thereafter developed, without compensation.

I grant P.O.I.N.T. the right to broadcast, market, sell and otherwise distribute my name, image, likeness, voice, appearance and performance, either in whole or in parts for commercial, non-commercial television, close circuit exhibition, home video distribution, internet, or any other purpose that P.O.I.N.T. in their sole discretion may determine.

I hereby give all clearance, copyright or otherwise, for the use of my name, likeness, image, voice, and appearance embodied in my/our performance at the production of "Rural American Idol" Program of People Organizing Ideal Neighborhoods Today, Inc., or any appearance and performances emanating from the "Rural American Idol" Program of People Organizing Ideal Neighborhoods Today, Inc., and its production of Rural American Music Revue.
